Applying over the internet is fast and easy. Navigating through this tutorial is simple, but you will need to read the instructions carefully. Red asterisks identify questions that are required to be answered. When your application is complete you will receive a confirmation number.
What you will need:
During your application process, you will be required to set up a user I.D. and password. There are also several things that you will need to have available when filling out your online application. They are as follows:
- Your social security card number
- Your last employer’s business name, address, and phone number
- First and last dates you worked for your last employer. (month, day, and year)
- The number of hours you worked and your pay rate if you worked during the week that you apply. (This includes Sunday)
- Military employment (service) start/end dates and a copy of your DD Form 214(S) (member copy 4-8) if you served in the military during the past 18 months
- Your alien registration number
